Note:  the following forms must be on file with Human Resources before you can use a state vehicle. Only fill out the third form if you need to notify Human Resources about any activities that may affect your eligibility to operate a motor vehicle for state business.

1) Driver’s History Consent Form
2) Driver Qualification Procedure Acknowledgement Form
3) Driver Qualification Procedure Notification

The forms must be signed and submitted to Human Resources every six (6) months.
